Dallas Horizon Home Health Agency, LLC currently accepts Medicare, Medicaid, Cigna/Healthspring, Amerigroup, Aetna Better Health of Texas, Blue Cross Blue Shield, Molina, US Department of Labor Workman's Comp, and Private Pay.
The Skilled Nurse is an RN or LVN with training and experience in providing care in the home. The nurse communicates frequently with your physician to ensure that your care is current and effective. The licensed and experienced nurses will perform a range of duties, including but not limited to medication managment, teaching and instruction on different diagnoses and disease process. Assessing you or your loved one for signs or symptoms that need to be addressed and reported to your physician.
Supplies/Therapy/Equipment: Medical Supplies, Durable Medical Equipment (DME): These devices may be required to complete your plan of care. While recieving services we are happy to coordinate the purchase and supply of medical supplies for our patients. DME (e.g., walker, wheelchair, hospital bed) is covered separately and may be supplied by the home health agency or an outside Medicare-certified DME supplier of your choice.
Physical, Occupational and Speech Therapy services are provided by licensed therapist or licensed therapy assistant under the direction of the therapist. Your therapist will provide specific information about the services and treatment you will receive.
